You can join the practice by calling our office at (813) 898-8980. You can also email us at admin@revivingmyhealth.com. If you email us, we will need your first name, last name, date of birth, email address, and a contact number.
We currently do not accept insurance and are a cash pay practice. Some laboratory testing and prescriptions are covered under some insurance policies. Please contact our office for more details.
Your first appointment will include a consultation to create your treatment plan. Once established as a patient, you can expect to have lab work and follow-up consultations every 3 months.
We will complete lab work to follow your progress throughout your treatment plan. Lab work is completed and reviewed every 3 months to make adjustments to any prescription medications or supplements.
Our practice offers a variety of extra services including functional medicine and clinical aesthetic counseling and procedures. Take a look around our website to see what we offer and contact our office with any questions or to schedule.
An established patient can get in touch with Dr. Eriksen or Carol Rodriguez, ARNP by calling the office or emailing us.
All patients have access to our Reviving My Health client portal. Simply login to your portal and review your records under the documents tab.
